A Tragic Crane Accident Took Two Lives In Massachusetts: Safety Concerns Rise

Even to the date, accident from heavy construction equipment wrong handling is still the reality. Despite a lot of safety regulations being imposed in the industry, fatal accidents are still reported, and people are still losing their lives. 

On Friday afternoon, a crane at the Exxon terminal on the Mystic River suddenly collapsed while workers were dismantling old dock equipment. The incident claimed the lives of two employees of a subcontractor under Charter Contracting: Paul Ledwell, 37, of Middleborough, and Larriston Lake, 44, of Roslindale.

A Routine Task Turns Tragic

A seemingly ordinary demolition project quickly turned into a fatal disaster. The enormous structure fell onto the docks as a result of the crane’s cables breaking during the operation, according to the Middlesex District Attorney’s Office. Within minutes, emergency personnel arrived, but one employee died on the spot, and the other died later in the hospital.

The Mystic River runs alongside the dock where the collapse happened, close to the Encore Casino and the proposed location of the New England Revolution’s soccer stadium. 

Authorities claim that the crane fell while the workers were dismantling antiquated equipment. 

This is very common in these types of accidents: during the dismantling of the equipment or while setting up the equipment for the job, people overlook the safety precautions as they think the machine is not working yet. However, this is as crucial a time to be looked at as during the machine running. The investigation becomes more complex when the used heavy equipment involved in the accident is huge and is more complex.  

The investigation is still under process

The cause of the collapse is being looked into by state and local authorities, such as the Office of the Chief Medical Examiner, the Everett Police Department, and the Massachusetts State Police. 

Occupational Safety and Health Administration (OSHA) officials were supposed to supervise the inspection, but their investigation has been postponed because of the ongoing federal government shutdown.

Although the exact cause has not yet been determined, preliminary reports indicate that the failure might have been caused by damaged crane cables or a load management problem. According to experts, crane cables can deteriorate under extreme strain and eventually break if they are not properly maintained or replaced after prolonged use, especially during demolition work where loads are unpredictable.

What do the industry and community think about this incident?

The entire city is with the victims’ families, friends, and coworkers, said Everett Mayor Carlo DeMaria, who offered his condolences. The city will fully support the investigation, he said, adding that our community mourns alongside all those affected by this tragedy.

Additionally, a statement lamenting the loss of their two members was issued by the Pile Drivers & Divers Local 56 union. 

The union underlined its continued dedication to safety, saying that every precaution must be taken to safeguard employees in hazardous situations, such as demolition and dockside work.

Is safety still overlooked at the construction sites?

This incident serves as a harsh reminder that safety is still the most important consideration on any job site, even with advancements in equipment technology. Such accidents are frequently caused by cable wear, rigging mistakes, and operator fatigue, all of which can be avoided with regular maintenance and safety supervision
